Senior Friendly Hospitals ACTION Program Cohort 1

Forty-four hospitals began their journey on the Senior Friendly Hospitals ACTION (Accelerating Change Together In ONtario) program, launched with a 3-day workshop on April 29 to May 1 2015 in Toronto. Teams were provided with knowledge-to-practice training on the SFH framework, quality improvement methods, and change leadership. They then returned to their hospital organizations to lead the implementation of a SFH project, while receiving ongoing coaching and continued networking via phone and webinar. Pictured is the workshop's clinical keynote speaker, Dr. Alison Mudge, Clinical Director of the Australian Centre for Health Service Innovation and developer of the "Eat, Walk, Engage" program created to improve care processes for hospitalized older adults.
