Introducing The Senior Friendly Care (sfCare) Framework

Have you heard about the new Senior Friendly Care (sfCare) Framework? The sfCare Framework, which builds on the successful Senior Friendly Hospital (SFH) Framework, was developed by the RGP of Toronto in collaboration with provincial stakeholders, including older adults, caregivers, and care providers. The goal of senior friendly care is to achieve the best possible health outcomes for older adults. The sfCare Framework provides the foundation for achieving this goal through guiding principles and defining statements directed at healthcare organization and providers. The framework is intended to foster improvements in care across the system and inspire greater collaboration between older adults and their caregivers, care providers, and organizations.
