Provincial Senior Friendly Hospital Summary Reports
In June 2011, LHIN-level Senior Friendly Hospital summary reports were forwarded to the RGP of Toronto for review. Working with all LHINs and RGPs across Ontario, a provincial summary report was generated. Building upon the promising SFH practices identified in each LHIN-level summary, an array of recommendations structured around the SFH framework was proposed to support hospitals in their work of becoming senior friendly. The report also identified three areas of practice for priority action: (1) delirium, (2) functional decline, and (3) transitions in care.
In October 2011, the provincial SFH Indicators working group, co-chaired by the RGP of Toronto and Baycrest, was convened. This group engaged 34 subject matter experts across Ontario to collaborate in a Delphi-consensus protocol designed to identify accountability indicators pertaining to delirium and functional decline in hospitals. A sub-working group is presently orchestrating a feasibility study of these indicators.
