SFH Environmental Scan 2014

In the fall of 2014, 135 hospitals across all 14 LHINs completed self-assessments which informed an updated environmental scan. The purpose of this update is to identify improvements in SFH commitment and care since 2011; facilitate organization- and LHIN-level planning of SFH activities; highlight new and existing promising practices; and identify training needs to build further SFH capacity. The Ontario summary and all LHIN-level reports are available below:

SFH Indicators Development and Evaluation

In October of 2011, a provincial SFH Indicators working group engaged 34 subject matter experts across Ontario to identify accountability indicators to drive improvement in hospital practice in delirium and functional decline.

In January 2013, the SFH indicators were implemented in a collaborative of 44 hospitals across 10 LHINs to evaluate their feasibility, clinical value, success factors and challenges.

SFH Environmental Scan 2011

The Ontario Senior Friendly Hospital Strategy, led by the Local Health Integration Networks (LHINs) and Regional Geriatric Programs (RGPs) of Ontario, was launched in winter 2011 with an environmental scan of hospitals across the province. The RGP of Toronto worked with all of the LHINs and RGPs of Ontario to generate a provincial summary of Senior Friendly Hospital care. This report provided twelve recommendations to help hospitals become more senior friendly, and identified hospital-acquired delirium and functional decline as improvement priorities for the system. Below we provide access to the SFH self-assessment template used in the scan; the LHIN-level summary reports generated by the RGP of Toronto; and the provincial summary report.
