In a Senior Friendly Hospital, care is designed from evidence and best practices that are mindful of the physiology, pathology, and social science of aging and frailty. Care and service across the organization are delivered in a way that is integrated with the health care system and support transitions to the community.

The enhanced Delirium and Functional Decline modules were developed by a "Senior Friendly Hospital Promising Practices" working group of the LHINs of Ontario, co-chaired by the RGP of Toronto and Baycrest. The modules consist of the following elements:
1. A topic overview
2. Tools for Screening and Detection,
3. Tools for Prevention and Management, and
4. Tools for Monitoring and Evaluation.
The remaining modules provide:
1. Clinical Tools
2. Learning Resources, and
3. Materials for Patients and Families.
